Care Planning & Consultations

The AFSC provides free, confidential care consultations for families living with Alzheimer’s and other dementia-related diseases, and can include any member of your care community.

These 30 to 60-minute consultations are based on the innovative work of New York University’s Caregiving Counseling Intervention and the Consultation Model of the Alzheimer’s Family Support Center. Service is provided via Zoom so that family members and friends can participate in conversation from anywhere they might be.

Consultations are by appointment only. Please call (508) 896-5170 for information or to schedule with one of our clinicians.
